Nursing Care of High-Risk Newborn- Development:Questions And Answers
A 38 2/7 weeks gestation, a newborn was delivered to a 25-year-old woman of Hispanic heritage with two
previous deliveries resulting in two living children. APGAR scores of 8/9/9. The mother was watched closely
during the last trimester to track the baby's size because her older children were on the smaller side. The
family history includes that the mother's sister had an infant die in utero in the first trimester. Diabetes and
obesity run in the family. All the women are about 5'2" or shorter. The father is 5'7", which is tall for his family.

The newborn is small and thin, with a wrinkled face and lusty cry.

A client gave birth at 42 3/7 weeks gestation. When assessing the neonate, which physical findings are
expected? Select all that apply.

Sleepy, lethargic baby

Lanugo covering the body

Long hair and nails

Creases cover the soles of the feet - ansDesquamation of the epidermis

Vernix caseosa not present

Long hair and nails

Creases cover the soles of the feet

--------------

Postdate fetuses lose the vernix caseosa and the epidermis may become desquamated. These neonates are
usually very alert. Lanugo is missing in the postdate neonate. Both the hair and nails have grown longer, and an
increased number of creases are visible across the soles of the feet.

A client with diabetes delivers a full-term neonate who weights 9 lbs, 10 oz.

While caring for this neonate, the nurse should palpate the (clavicle or hips) after delivery. It is also important
to check for (symmetry or limb length) to see if there has been any nerve damage. - ansClavicles

Symmetry

Because of the neonate's large size, clavicular fractures are common during delivery. The nurse should assess
all large for gestation age (LGA) neonates for this occurrence. With nerve damage, the provider will observe if
there is any issue with symmetry.

A newborn delivered at 38 2/7 weeks is considered (preterm or term). A weight at the 8th percentile for age is
considered intrauterine growth (restricted or small for gestational age) - ansTerm

Small for gestation age

----------

Term infants are those born after the beginning of week 38 and before week 42 of pregnancy. Infants born
before term (before the beginning of the 38th week of pregnancy) are classified as preterm infants, regardless
of their birth weight.
