1. What would your priority nursing diagnosis be prior to meeting the client?
Impaired skin integrity R/T altered epidermis and/or dermis AEB Stage 3 Diabetic Ulcer on clients’
sacrum.
2. Ask Thomas 10 questions that you would like to ask him during your first meeting with him.
1. What is your name?
2. What is your DOB?
3. Do you have any allergies to medications, food, or environmental factors?
4. What brings you in today?
5. What time is it?
6. Where are you?
7. How would you rate your pain on a scale of 0-10, 0 being none and 10 being the worst?
Then proceed with PQRST if pain is rated numerically.
8. Are you currently taking any medications?
9. Do you have any other health concerns you would like to discuss?
10. Do you smoke and/or drink alcohol?
3. Write out your plan of care (interventions/goals) you would like to share with Thomas, to see if
he agrees.
Interventions: -Wash hands thoroughly before providing wound care on the client
- Assess the client’s wound for color, odor, condition of wound edges, exudate,
and measure the dimensions of the wound
-Provide wound care and dressing
- Assess the client’s nutritional status
- Assess the client’s level of pain
- Administer medication for pain as ordered by the physician
