DIGFAST – Bipolar disorder acronym D- distractibility I-impulsivity G-grandiosity F-flight of ideas A-activity, hyperactive S- sleep; requiring less T- talkativeness BPD type 1 timing - mania lasts at least 1 week rapid cycling definition - at least 4 cycles per year with 8 weeks of well periods BPD 2 definition - current or past hypomanic episode AND current or past major depressive episode cyclothymia definition - episodes of weeks to months of being happy and energetic followed by months of depression and irritability; symptoms not severe enough to impact functionality cyclothymia timing - at least 2 years in adults and 1 year in kids with periods of hypomania that do not meet criteria for hypomania; present at least 1/2 the time and not without longer than 2 months cyclothymia treatment - antipsychotics and antidepressants cyclothymia patients at higher risk for - bipolar disorder dysthymia definition - at least 2 years of chronic mild MDD symptoms dysthymia treatment - antidepressants and therapy patients with dysthymia at higher risk for - MDD hypomania definition - present for at least 4 days and less than 1 week; not severe enough to cause impairment in social or occupational functioning; no psychosis modifiable risk factors for suicide - living alone, unrealistic parental expectations, abuse, mental illness, substance abuse, poor coping skills, chronic severe stress nonmodifiable risk factors for suicide - being male, family history of MH or SI, race, gender, previous suicide attempt loss through death or divorce, bad parenting, sexual orientation, gender identity % of suicides male - 78% suicide attempt rate in population - 2.5% criteria for involuntary hospitalization - 1. mental illness, developmental disability, or drug/etoh dependance 2. the illness/disability/dependance is treatable 3. the person is dangerous to him/herself or others due to the illness/disability/dependance *danger to self/others or unable to care for self possible medical conditions that can mimic Mental illness - hypothyroidism, diabetes what is bupropion good for - ADHD, depression, sexual side effects when should you not use buproprion - head injury or seizure carbamazepine birth defect - spina bifida/neural tube defects **folic acid carbamazepine target - 4-12 carbamazepine starting dose - 200mg BID carbamazepine side effects - SJS rash, cardiac, CYP450! carbamazepine not good for who - pregnant older adults (due to med interactions) fluoxetine not good for - elderly d/t CYP 450
