MSN 572 Acute Pharyngitis Soap Note with Instructor: Dr. Nakeda Hall 100% Correct ID: J.C. born on 06/24/1990, 34 y/o, Asian female and non-Hispani. J.C. is alert and oriented in time, situation, name and person. Patient came to the clinic by herself and drove alone. HISTORY OF PRESENT ILLNESS (HPI): J.C. came at the clinic with complains of sore throat that has been experiencing it 3 days and worsens in the morning. The patient denies any exacerbating circumstances. J.C. reports that warm liquids help her throat feel less painful. The patient reports that, despite attending work, she is experiencing increasing weariness and suspects her health is worsening. J.C. reports that her throat hurts on a 6 out of 10 pain scale. The patient denies seasonal allergies. Pt is a healthy person without any health issues and an excellent historian. Two weeks earlier, the patient, who was a new patient, had a fantastic examination and showed no signs of acute sickness or complaints. She only feels relief from two hours of Tylenol's benefit to her. She does, however, observe that as the prescription wears off, her symptoms deepen, and she feels worse than before. She denies having a fever, cough, or congestion in her nose.
