perineal - ANS the region between the anus and external genitals
perianal - ANS area around the anus
steps to changing linens - ANS 1. roll the pt to one side of the bed
2. tuck the sheets to be removed under the pt
3. replace the sheets on the unoccupied side
4. tuck the fresh sheets under the pt
5. roll the pt to the fresh side
, 6. remove the dirtied linens and pull the fresh linens to the unoccupied side of the bed.
7. return pt to their original position
signs of active listening - ANS smile, eye contact, erect posture, attention to the speaker
signs of hypoxia - ANS anxiety, lack of concentration or focus, fatigue, hypertension, cyanosis, dyspnea,
change in heart rate or respiratory rate
hypoxia - ANS Low oxygen saturation of the body, not enough oxygen in the blood
when to reposition a pt - ANS at least every 2 hours
indications of wound infection - ANS redness, swelling, increasing pain and discomfort, warmth around
the wound, fever, foul odor, drainage
expected hr for an adult - ANS 60-100bpm
expected respiratory rate for an adult - ANS 12-20/min
five rights of delegation - ANS right task, right circumstance, right supervision, right person, right
