Chronic Diarrhea - Answers Passage of loose stools with increased frequency that lasts for more than 2 weeks, often caused by chronic conditions, i.e.; malabsorption, IBD, food allergy, lactose intolerance Factors that impact diarrhea - Answers Age, impaired health, climate, environment Primary observation-Diarrhea - Answers Assess for noticeable or sudden increase in number of stools, reduction in consistency with increase in fluid of content. Stool greenish in color -indicates bile that was not reabsorbed by the body(rapid progression through GI) fever, vomiting, fretful, irritable, borborygmi-may indicate mechanical obstruction in large intestine, Dehydration-sunken fontanel, eyes, no tears, muscle aches-d/t electrolyte loss. Secondary observation - Answers blood cultures, blood serum-botulism, Sigmoidoscopy- colitis vs. parasite etc.. Dx test for stool analysis - Answers pH level, Gualac test: Neutrophils or RBS's =bacterial enteritis or IBD, Eosinophil's=protein intolerance or parasite. Bacteria, Glucose=malabsorption, Fat=malabsorption, C&S (organism detection, cross & match with right ATB) Ova/parasites, Hct (concentration will be high b/c loosing fluid volume d/t high concentrate RBS's), BUN (b/c filtration is altered) b/c kidney unable to excrete waste products Electrolyte profile (Na+) - Answers Isotonic dehydration Na+ normal (130-150) Hypotonic dehydration Na+ <130 Hypertonic dehydration Na+>150 serum CO2 - Answers Serum CO2: normal 20-28 Early sign of dehydration below 20 indicated dehydration and needs hospitalization. Diarrhea consist of alkaline stools . With alkaline leaving the body, pH is decreased and the body goes into metabolic acidotic state. Serum CO2 level is a reflection of the bicarbonate, there for CO2 levels are low. Dehydration - Answers Occures whe total output of fluid exceeds total intake. The younger the child the bigger the prob. Infant is 80% water son small decrease can cause big problems! Age 3 is 60% water.
