How long till a patient has to have a referral from their primary care provider? 21 days or 8 visits from the initiation of a physical therapy plan of intervention. What are additional requirements for seeing a patient who has self-referred? A doctorate in physical therapy or equivalent degree from an accredited institution plus two years of clinical practice experience, post grad certification, a specialty, residency or fellowship training.
