Abdomen LOC (Level of Consiousness) *Time *Place *Person Skin *Symmetry *Everything Normal Head [CN5, CN7] *Hair [texture, color, thinning] *Palpate the skull [lumps, bums?] *Scalp [clean, seborrhea] if patient has wigs/ scarfs, just ask the patient questions Face *Skin [color, cyanotic, pale] *Nose symmetrical (No drainage, assess for patency, can they smell [CN1] *Mouth [lips; symmetrical, pink, moist look in mouth; uvula midline, mucous membranes pink & moist, tongue pink and moist tongue smooth in front, bumpy toward the back] ^Does person have all teeth, any missing teeth [This can affect nutrition if person cannot eat] *Nose [nares; septum symmetrical & pink] Eyes [CN3,4,6] *Inspect [eyelashes, sclera, conjunctiva, drainage of the eye, the pupil] *PERRLA [Pupils, Equal, Round, Reactive to Light, Accommodation] lights in room should be out *During accommodation, when item further [pupils are dilated], when item closer [pupils constrict] ^Eyes constrict and conversion (crossed eyed)
