CCEP clinical practices Weber's test - correct answer - A vibrating tuning fork (512Hz) is placed in the center of the forehead at the top of the patient's head, equidistant from the patient's ears. The patient is asked to report in which ear the sound is heard louder. - In a normal patient, the sound is heard equally in both ears (no lateralization). However, a patient with symmetrical hearing loss will hear the sound equally as well. Thus, there is a diagnostic utility only in asymptomatic hearing losses. - In an abnormal patient, the sound is heard louder in one ear (lateralization). This should be confirmed by repeating the procedure and having the patient occlude one ear with a finger by wiggling the tragus. The sound should be heard best in the occluded ear. i.e. If the patient says they can hear it louder on his/her right ear, then that means something is blocking the sound waves traveling through the air. The "quieter" ear is receiving interference from the sound traveling through the air so the sound traveling through the bone is minimized What is positive sign in Weber's test? - correct answer The sound is heard laterlization. The sound should be heard best in the occluded ear Rinne's test - correct answer - It compares perception of sounds transmitted by air conduction to those transmitted by bone conduction through the mastoid process. Thus, one can quickly suspect conductive hearing loss. - The Rinne Test is performed by placing a vibrating tuning fork (512Hz) against the patient's mastoid bone and ask the patient to tell you when the sound is no longer heard. Time this interval of bone conduction with a watch, noting the number of seconds. Quickly position the still vibrating tines 1-2cm from the auditory canal, and again ask the patient to tell you when the sound is no longer heard. Continue timing the interval of sound due to air conduction heard by the patient. Compare the number of seconds sound is heard by bone conduction versus air conduction.
