The International Civil Aviation Convention, if a contracted state aircraft is in danger of being involved in an accident or any other incident in another contracting state, the precarious state is required to launch an investigation. This convention establishes the rights and obligations of the state in certain situations. The Swiss cheese model remains based on why companies must work together to provide safe and effective operations. According to Carayon and Hoonakker (2019), Organizations must collaborate to ensure safe and successful operations. For pilots, maintaining a safe flight operation requires that all human and mechanical aspects of the system function successfully together as a whole. The hole in the causal model symbolizes a weakness or failure. Fortunately, the presence of protective layers means that these holes will not harm the environment. Nevertheless, the danger will emerge once the openings have remained filled in. Backhouse and Malik (2019) state that this model contains four patterns: organizational influence, harmful controls, preconditions, and hazardous actions. The organizational impact is the first of these patterns. Climate resource management, organization, and administrative processes fall under "organizational influence." For example, a crew that underestimates the expense of maintenance will leave the aircraft and its equipment in poor condition after the flight. On the other hand, Insecure Controls include insufficient monitoring, unsuitable operation, inability to address problems, and violations of oversight procedures. For example, failing to provide new staff with immediate training will raise the likelihood of suffering a life-threatening injury. According to Carayon and Hoonakker (2019), unsafe action does not always result in physical harm. Specific conditions must remain met before engaging in harmful activities; for example, an unstable mental state contributes to poor decision-making. Mistakes and Abuse are terms used to describe dangerous behavior or involve hazardous action. People who make mistakes cannot take the appropriate action to achieve desired objectives. Acts of defiance of the law or rule are considered abuses. These four layers are the essential components of the Swiss cheese model, and risk analysis may remain carried out by keeping track of all of these aspects simultaneously.
