Dysrhythmia Review
Dysrhythmia ReviewDysrhythmia Review Dysrhyth mia What’s Wrong Etiology What’s Happening in the Heart Symptoms How Do We Fix It? Normal Sinus Rhythm (NSR) Nothin g. - Everything is normal, HR 60-100 bpm & all intervals are normal None. We don’t have to. Sinus Tachycard ia (ST) HR 100- 150 Pain, Stress, Medications, Caffeine, Exercise, etc. Beating too fast Poor cardiac output, if symptomatic 1. Stop cause 2. Beta blockers 3. Vagal down (rare) Sinus Bradycar dia (SB) HR 60 bpm Cardiac meds, heart disease, Sleep, welltrained athletes Heart is beating too slow Poor cardiac output (dizziness, weakness, lethargy, nausea, confusion, syncope) 1. Atropine 2. Pacemaker 3. Stop offending drugs Atrial Fibrillatio n (afib) Atria are vibratin g Cardiac disease (CAD & CHF) Idiopathic Atria are vibrating, not providing the normal CO – blood coagulates, causing clots If any, palpitations, hypotension, fatigue, syncope, shortness of breath, & angina (due to loss of CO) 1. Medications (BB/digoxin/amio ) 2. Cardioversion 3. Ablations 4. Anticoagulants to treat the clots & prevent MI, CVA, & PE’s This study source was downloaded by from CourseH on :52:15 GMT -05:00 onlystudents Dysrhythmia Review Atrial Flutter (aflutter) Atria are flutteri ng Cardiac disease (CAD & CHF) Idiopathic Atria are vibrating, not providing the normal CO – blood coagulates, causing clots If any, palpitations, hypotension, fatigue, syncope, shortness of breath, & angina (due to loss of CO) 1. Medications (BB/digoxin/amio ) 2. Cardioversion 3. Ablations 4. Anticoagulants to treat the clots & prevent MI, CVA, & PE’s Supraven tricular Tachycard ia (SVT) HR 150 Overexertio n, stress, stimulants (cocaine/ca ffeine), digoxin toxicity Ectopic (outside of the SA node) beat throws the rhythm 150 bpm Profound loss of cardiac output (syncope, angina, SOB, hypotension) 1. Vagal stimulation 2. IV adenosine (slam) 3. BB/CCB’s 4. Amiodarone 5. Cardioversion Ventricula r Tachycard ia (VTACH) HR 200 Heart disease, electrolyte imbalances (K), medications / drugs, Ectopic ventricular beats take over as the pacemaker of the heart With a pulse: Profound loss of cardiac output Without a pulse: Apneic, asystole With a pulse: - Cardioversion - Antidysrhythmics - Treat cause Without a pulse: - Defib, CPR, epinephrine, & amiodarone This study source was downloaded by from CourseH on
