They are helpful in pts with comorbidities such as heart failure and diabetes. Angiotensin receptor blockers (ARBs) block the effects of a hormone called angiotensin II in your body. When angiotensin II enters your blood stream your blood vessels become narrower. This gives your blood less space to move in, which raises your blood pressure. Angiotensin II also triggers a hormone that makes your body retain water. Having more fluid in your body, in a restricted space, will cause your blood pressure to rise. Angiotensin receptor blockers (ARBs) lower your blood pressure by blocking the actions of angiotensin II. This allows your blood vessels to relax and widen, making it easier for blood to flow through. It also reduces the amount of water your body retains, which lowers your blood pressure. THIAZIDE DIURETIC Nonblack thiazide-type diuretic Age over 60 blood pressure goal SBP 150 DBP 90 age 60 SBP goal 140 and DBP all ages with diabetes present no CKD blood pressure goal SBP 140 and DBP 90 initiate thiazide-type diuretic or ACE or ARB or CCB, alone or in combination. Maximize first medication before adding a second or add a second medication before getting to the max does of first. If not at goal BP continue to titrate the thiazide diuretic. Stage I
