Nursing Care Plan Small for Gestational Age Nursing Diagnosis Inability to regulate core body temperature related to lack of subcutaneous fat due to SGA as evidenced by body temperature of 36OC an d low birth weight. Nursing Inference Hypothermia may occur because of impaired thermoregulation, which involves multiple factors including increased heat loss due to the decrease in subcutaneous fat, decreased heat production due to intrauterine stress and depletion of nutrient stores, and increased surface to volume ratio due to small size. SGA infants should be in a thermoneutral environment to minimize oxygen consumption. Nursing Goal After 4 hours of providing nursing intervention, patient will display core temperature within normal range. Nursing Intervention Intervention Rationale • Monitor vital signs. • Encourage mother to hold baby for skinto-skin contact. • Keep baby dry and wrapped in a wrapped in a blanket; keep the blanket; keep the infant’s head covered with cap. • Note and measure patient’s body temperature. • Serves as a baseline information and any changes may indicate worsening of child’s condition. • Skin-to-skin care was shown to be effective in reducing the risk of hypothermia when compared to conventional incubator care for infants. • To prevent a further decrease in body temperature • To assess effectiveness of effort Nursing Evaluation After 4 hours of providing nursing intervention, patient was able to meet and maintained stable core temperature within normal range with no signs of respiratory distress and skin warm to touch. Nursing Care Plan Large for Gestational Age Nursing Diagnosis Imbalanced nutrition: more than body requirements related gestational diabetes as evidence by weight. Nursing Inference Diabetes in the mother is the most common cause of babies who are large for gestational age. When a pregnant woman has high blood sugar, she can pass that along to her baby. In response, the baby's body makes insulin. All the extra sugar and the extra insulin that is made can lead to fast growth and deposits of fat. This means a larger baby. It also means a risk for low blood sugar right after birth. At that point, the mother's supply is no longer there, but the baby's insulin levels stay high.
