Diagnostic and Statistical Manual of Mental Disorders
DSM = Beschrijvend.
Het systeem beschrijft symptomen van afwijkende gedragingen en is geen poging tot het
verklaren van de oorzaken daarvan.(de DSM is dus atheoretisch).
Bij diagnosticeren is het mogelijk iets te zeggen over mogelijke oorzaken, bij classificeren
niet.

Bedoeling: ambitieus onderzoeksplan om diagnostische besluitvorming aan te sturen
vanuit baanbrekende resultaten van biomedisch onderzoek, dit zou dan kunnen leiden tot
een meer objectieve afbakening van diagnostische categorieën.
Het nieuwe handboek moet voortbouwen op wetenschappelijke innovatie, gebaseerd op de
nieuwste wetenschappelijke inzichten die dan leiden tot objectieve afbakening van de
diagnostische categorieën.”
De hooggespannen verwachtingen na 2 decennia psychiatrisch en neurobiologisch
onderzoek werden niet ingelost.
Biologische inzichten hebben wel geholpen, maar niet geleid hebben tot veranderingen

Voorloper DSM: ontstaan nav een volkstelling in begin van 20 ste eeuw: ‘Statistical manual
for the Use of Institutions for the Insane’
Opkomen van ambulante praktijken, vnl. in handen van neurologen (zenuwartsen).
Overwaaien van ideeën uit Europa (Freud).
Na WO I en II werden psychiaters geconfronteerd met diagnoses die niet te plaatsen
waren in de vorige nomenclatuur
1952: DSM I uitgebracht: overwegend psycho-analytisch
Alles werd geplaatst op een angstspectrum
bv. angstreactie, schizofrene reactie
DSM II nog meer psychoanalytisch, terugkeer ‘neurosen’ en term ‘hysterie’.
In de jaren 70 kwam de diagnostiek zwaar onder druk: bv in VS werd de diagnose 2 keer
zo veel gesteld als in UK (en dat was nog voor de brexit)
