He is experiencing auditory hallucinations, increased irritability, repetitive behaviors that he says are to “keep him safe” such as checking locks and windows, and has showering and changing clothes. His mother reports he has stopped doing his usual activities and has cut off contact withalmost everyone he used to be friends with. She says he is not the same person she used to know. He stopped taking his ADHD medication (methylphenidate) 6 months ago because he thought it to be unnecessary and a manipulation technique. On exam, he was found to repetitively tilt his head to the left and scratch behind his ear; he also had multiple scalp excoriations of multiple ages. He is mildly obese.
