In this paper, I will be developing a final care coordination plan for a patient who is pregnant and dealing with an opiate addiction. I will design patient-centered interventions and timelines for a pregnant patient with an opiate addiction. I will consider ethical decisions in designing patient-centered health interventions. I will identify relevant health policy implications for the coordination and continuation of care. I will describe priorities that a care coordinator would establish when discussing the plan with a patient. I will use the literature on evaluation as a guide to compare learning session content with best practices. When coordinating care for an opiate addicted pregnant mother, there is often comorbidities that coincide with the addiction that need to be addressed at the same time. Because of this, there are usually different specialties that have to work together for the sake of the mother. A patient comes to the hospital when she is arrested on the street for her addiction.
