Shape/Contour (view finger at profile sign) - “Nail bed is flat” - “Nail folds are smooth and rounded” - “Nail base is 160 degrees” 2. Consistency - “Nail surface is smooth and regular” 3. Color - “Nail bed is pink” B. Palpation 1. Capillary refill (press edge of nail bed for a few seconds and release) - “Color returns within 1-2 seconds” - “Good capillary refill” II. SKIN A. Inspection 1. Color - “General pigmentation is even and consistent with genetic background” - Freckles - Moles - Birthmarks 2. Moisture - “No moisture present” 3. Hygiene -“Skin appears clean and well groomed” 4. Lesions “No lesions, skin is intact” 5. Masses -“No masses” 6.
