1. FMCSA, NRCME & the Medical Examiner Role
• FMCSA is the federal agency responsible for commercial motor vehicle safety regulation. Its mission centers on reducing
crashes, injuries, and fatalities involving large trucks and buses.

• The National Registry of Certified Medical Examiners (NRCME) is the federal registry for certified medical examiners who
perform qualifying CMV physical examinations.

• A medical examiner (ME) must meet applicable professional/licensure requirements, complete required training, pass the

• Abnormal findings do not automatically equal permanent disqualification. Determine whether the driver meets the standard,
requires additional evaluation/documentation, can be certified for a shorter interval, or does not meet the standard.

4. Cardiovascular Conditions
• Assess conditions that may cause sudden incapacitation or impaired exercise tolerance, including ischemic disease,
myocardial infarction history, arrhythmias, heart failure, hypertension, structural disease, and relevant procedures.

, • Focus on stability, symptoms, treatment response, functional capacity, recurrence risk, and whether required
documentation/testing is available.

• Hypertension questions often test the difference between an abnormal measurement and the regulatory/clinical decision that
follows. Repeat/confirm measurements appropriately and apply the current standard.

• High-yield principle: a cardiovascular diagnosis alone is not the whole decision. Ask whether the condition creates a safety
risk and whether the applicable standard is satisfied.

5. Neurologic Disorders & Seizure Risk
• Neurologic conditions matter because sudden impairment of consciousness, cognition, coordination, strength, sensation, or
reaction time can endanger the driver and public.

• Assess seizure history carefully: type, provoking factors, recurrence, treatment, medication adherence, neurologic
evaluation, and documentation.

• Stroke/TIA history requires careful attention to residual deficits, recurrence risk, treatment, and the applicable qualification
criteria. Do not assume a clearance letter by itself resolves the federal qualification decision.

• Exam trap: distinguish a medical specialist's opinion from the ME's responsibility to make the certification determination
under the applicable federal framework.

6. Diabetes Mellitus
• Assess diabetes treatment, glucose control, hypoglycemia history, complications, monitoring, medication adherence, and
functional effects.

• Insulin-treated diabetes is governed by specific federal requirements. The current rules should be checked directly rather
than relying on older study materials.

• Potential safety concerns include severe hypoglycemia, impaired awareness, visual complications, neuropathy, renal
disease, and other complications affecting safe operation.

• Exam focus: treatment with insulin is not itself a shortcut to an answer. Apply the current regulatory pathway and
documentation requirements.

7. Vision Standards
• Vision assessment is safety-critical because commercial driving depends on visual acuity, field of vision, color recognition,
and adequate functional vision.

• Know the difference between testing an individual visual field/eye and considering the driver's overall qualifying visual status.
