CHAPTER ONE: INTRODUCTION TO MIDWIFERY CASE SUDY



It is care given to a pregnant woman which is goal oriented where by a client whose gestation is between 28 to 32
weeks is identified and followed antenatal till six weeks postnatally.
Antenatally, the client is followed to monitor progress of pregnancy in order to ensure good maternal health and fetal
development. Here, possible complications are identified early and essential management taken into consideration.
During labor the mother is observed for progress of labor and given psychological and emotional support to prevent
complications to the mother and the fetus.
Postnatally the mother is followed to promote the physical wellbeing of the mother and the baby by advising the
mother on nutrition, hygiene, immunization and family planning.

✔​ It’s a state of woman when she carries developing embryo or fetus.


✔​ It can also be described as the time during which a fetus develops inside a woman’s womb or uterus. (Myles

DIAGNOSIS OF PREGNANCY



Sign and symptoms

1.Presumptive signs

◆​ These are the symptoms that the woman experiences and reports. They include:


✔​ Amenorrhea – missing of more than one menstrual period especially in a woman whose cycle is regular.

, ✔​ Nausea and vomiting – usually occur in the early mornings during the first weeks of pregnancy. It’s usually

spontaneous and subsides in 6-8 weeks or by the 12th – 16th week of pregnancy.

✔​ Frequent urination – it’s caused by pressure of the expanding uterus on the bladder. It subsides as pregnancy

progresses and the uterus rises out of the pelvic cavity.

✔​ Breast changes – they include temporary enlargement of the breast, development of Montgomery tubercle and

darkening of the areola.

✔​ Quickening – also called the feeling of life. It’s the first perception of fetal movement within the uterus.

2.Probable signs of pregnancy

✔​ Chadwick’s sign – it’s the bluish – purplish coloration of the vagina due to increased vascularity and is noted at

the 6th week.

✔​ Leukorrhea – it’s the increase in the whitish mucoid discharge due to hyperplasia of vaginal epithelial cells of

the cervix because of increased hormone level from the pregnancy.

✔​ Hagar’s sign – it’s the softening of the cervix.


✔​ McDonald’s sign – its increased ease of flexing the body of the uterus against the cervix.


✔​ Progressive increase in fundal height.


✔​ Braxton – Hick’s contractions can be palpated.


✔​ Positive immunological pregnancy test in early morning specimen from the 14th day after fertilization.


3.Positive changes

✔​ Fetal heart heard and is regular on auscultation.


✔​ Fetal movements are reported by the mother and can be seen through the abdomen.


✔​ The fetus is seen through ultrasound and x-ray.
