A1: Assessment Overview TYPE: online discussion forum
PURPOSE: Help determine the future teaching needs of the
student prior to the completion of this course. This will assess
how well the students have achieved the student learning
outcomes and identify any concepts that need clarification. This
will allow the educator to identify strengths and weaknesses for
the learner. The assessment will provide feedback on how well
the student is retaining the knowledge provided to them.
OUTCOMES: Students will be able to focus education to all of
those in the community, including those of different cultural
backgrounds. They will be knowledgeable of the education
needs pertaining to health promotion, disease prevention and
preventative care. They will be able to develop interdisciplinary
plans that will benefit the patient’s needs.

Course Objectives By the end of the course the students will:
1. Focus education to those in the community, individuals,
families, and groups of different cultural backgrounds,
about health promotion, disease prevention, and
preventative care.
2. Apply a comprehensive assessment to a multitude of
individuals, families, and groups in population.
3. Develop an interdisciplinary approach in decision
making, patient care efficiency, cost effectiveness, and
population focused interventions.

2. Be able to identify different approaches to educating
individuals and groups in a population.

SECTION B: FORMATIVE ASSESSMENT INSTRUCTIONS (student facing)
B1: Assessment Instructions
You will participate in online discussion boards throughout the course. You will use your online
student portal to log into the discussion board. You will click on the discussion link in the Week
3 module, Nursing practice at the local, state, and national level. You will read the scenario and
answer the questions with a two-three sentence answer. You will be expected to reply to two (2)
of your classmate’s answers. Your responses back to your classmates should also be 2-3
sentences and be constructive, non-argumentative, and provide feedback. Your answers and
your responses must be posted on Thursday night by 2359. Your responses to your classmates
must be posted on Friday night by 2359. The responses to your classmate’s should be creative
and positive.

Students will be expected to participate in the online discussion following the weekly lecture.
They are to follow the instructions provided for them.
SCENERIO:
You work in a busy community health clinic with most patients with different that are low socio-
economical, non-English speaking, low-educated, and/or of a different culture. Many of the
served community has not been following up with lab work, appointments, medication usage,
and/or well-health check-ups. The lack of care may have been increasing emergency room visits
and bringing patients to the Community Health Center in deteriorating conditions. You have
been tasked with the job to perform a survey to discover what would be the best education,
care, etc. to the community that is being served by the community health clinic. You will be
tasked with discovering the best ways to incorporate new ideas to entice patients to attend to
their health care needs. You will need to research, perform surveys with patients, and
investigate the best ways to provide care.
Answer the following questions and post them on the online discussion board following the
instructions listed above:
1. Put together a plan to address barriers that the patients may encounter to their health
care.
2. What could the nurse have done prior to the research to encourage patients to use the
community health center?
The responses from the students will allow the educator knowledge on whether they have

nursing practice at the local, state, and national level.
