Client and Caregiver Interview 2 minutes 58 seconds To plan for our therapy session, please tell me a little about yourselves. My name is Sherl, and I'm Dan's wife of almost 47 years. And I'm his companion and his caregiver also. Dan was a teacher in public school for 28 years, along with being a teacher in a college for 1 8 years. Some of that was concurrent. Dan: Yeah. Spouse: And after that he worked at other jobs. When he retired, he worked at the farmer's market for some of the vendors and had fun doing that right? Dan: Yeah. Spouse: And he worked in a bakery. Dan: Yeah. Spouse: Yes, and I think that's about all you did, that was enough. Dan's a Master Gardener. And he loves getting his hands in the soil, which he's been doing lately. Dan Yeah, [unintelligible]. 4 minutes 1 seconds Tell me about the story of your stroke. Dan had his first stroke... It was right after my father had passed away, right after we had to put my mother in the hospital. And then ten days later, Dan had a stroke. It was after several mini strokes. His stroke wasn't detected right away. He ended up in the hospital, but they didn't know what was wrong until nine days later. Because Dan has a pacemaker, they couldn't do MRIs, so they did CT scans and it, the stroke, didn't show up. So he was treated in the hospital. He was in ICU for sixteen days and then he was in therapy for about thirty days. But his first stroke, he recovered quite well. Recovered his speech, recovered his memory, and pretty much went back to work for a while. But then in... Ten days after our son's wedding, and he was best man at our son's wedding, but ten days after, he had this stroke. He had his second stroke, and then shortly after, he had a brain bleed also. But I came home from work that day and found him sitting at the table, the dog right at his feet. And we put together some of the pieces, I found groceries in the car. This was, I found him shortly after five o'clock. But I found groceries in the car, and the slip was from about 1 0:30 so. From what we can put together, is that he got into the house and sat down and stayed there until I came home about 5:30 or so. And I knew right away something was wrong. You know, he was smiling when he greeted me, but he didn't say a word, he didn't say anything. So I called the ambulance, and they took him to the hospital. I knew right away what was wrong, I knew that he had had another stroke. And then he was treated in the hospital for, he was there for thirty-one days and then transferred to a rehab facility with occupational therapy, physical therapy, and speech therapy. And he's pretty much been in therapy since then. We go pretty much wherever we can to get him therapy to continue. And his progress is continuing, even after what, four years. 7 minutes 38 seconds What has become more challenging for both of you since the stroke? Of course, the communication is a little difficult. And sometimes it takes some effort to get the point across, both of us. Sometimes he doesn't understand what I'm asking him. Dan: [laughing]. Spouse: Sometimes I think he ignores me. Dan: [laughing]. Spouse: And sometimes he'll want something and I have a hard time understanding that. We do a lot of twenty questions. Do you mean this, do you mean this? Dan: Yeah. Spouse: And he'll say no, no, no, no, yes. Dan: No. [laughing] Spouse: Right? What else do you find challenging? You take care of the dog, pretty much. Dan: Oh yeah. Spouse: Except when he gets wrapped around a tree. Dan: Yeah, yeah, yeah. Spouse: He yells for me. And then I don't know what happened last night that the garage door got shut and the dog got shut in the door. I don't know what happened with that. Dan: [unintelligible]. Spouse: We fixed it. 9 minutes 38 seconds What types of strategies do you both use to improve communication? Dan:[unintelligible]. No, no, no. Spouse: I’ll ask you a lot of questions, right? But also, the one time, the one time, oh I know what it was, the one time he was trying to tell me something, and I just couldn’t get it. He went to the phone book and he pointed to what he wanted in the phone book, which he wanted to go to a car dealership, and I thought that was, that was pretty good, that he could do that. And sometimes you know he’ll point to pictures, sometimes he tries to write it and that’s not, we’re not real successful with that, but he tries. He’ll add things to my grocery list and then when we get to the grocery, I said okay, you go get that. Dan: Yeah. Spouse: He’ll go get that. Right? Dan: Yeah. Spouse: But we try different things. We do have some of the things that we’ve made in therapy that are helpful. Dan: Mmm-hmm. Spouse: Yesterday, when we went to stroke support group, we have a sheet that one of the therapists, the student therapist, had made and it says his name and when he had his first stroke and when he had his second stroke. And those kind of help trigger his, his well, his memory and his speech. We try a lot of different things. The one time he wanted to go someplace and I could not figure out where he wanted to go. I said okay, you tell me where to turn, and we ended up in Davison, and I think maybe that is where he wanted to go, but I don't do that very often.
