This scenario presents some obvious value conflicts that have been discussed in the APHA Code of Ethics. First, Human Rights and Civil Liberties requires that personal autonomy, privacy, self-determination, and freedom from domination be respected by ensuring the confidentiality of health information (American Public Health Association, n.d.). Conversely, the principles of Health Justice and Equity and Health and Safety call for broadening access to essential services among vulnerable communities to foster well-being and decrease inequalities (American Public Health Association, n.d.). Further, concerns over Professionalism and Trust only add to this dilemma since any violation may cause loss of public trust. According to Solimini et al. (2021), the privacy challenges associated with telemedicine, including security breaches and unexpected exposure in remote environments often conflict the requirement of equitable access, particularly during times of high demand. A2: Justification of Domains The two domains from the APHA Code of Ethics that will inform a plan to solve the scenario A ethical challenge are Domain 3: Inform and educate the public about health issues and functions, and Domain 7: Promote improved access to social resources conducive to health and health care (American Public Health Association, n.d.). Domain 3 can contribute to resolving the privacy-access challenge through transparency and accuracy in communicating with patients and communities about the advantages, risks, and safeguards of telemedicine systems. This domain aids in restoring professionalism and trust, while also dealing with issues of privacy through risk communication and community involvement, 3 which is very important after any breach of confidence due to data breaches (American Public Health Association, n.d., 4.3.1-4.3.3). This also ensures a compromise between conflicting values by making informed decisions as far as autonomy and organizational goals are concerned. Domain 7 is relevant because it deals with equal access to resources that contribute to better health and well-being, including digital health, and stresses the importance of reducing disparity and respecting dignity. This domain ensures that telemedicine is expanded while respecting privacy, addressing the issues of Health Justice and Equity along with Human Rights and Civil Liberties (American Public Health Association, n.d.). The domains would balance the interests of privacy protection and fair healthcare provision, as illustrated in the scenario (Mishkin et al., 2023). A2a: Domain Suitability Domain 3 (Inform and educate the public about health issues and functions) is very appropriate since it provides the organization with useful means to deal with erosion of trust following data breaches and ensure that patients make informed decisions. The domain demands public health professionals to be “honest and accurate” when discussing real and possible problems, as well as highlighting strengths and weaknesses, and involving everyone in the community (American Public Health Association, n.d.). This domain provides clear guidelines for scaling up telemedicine safely without increasing the digital divide or compromising on privacy, thus achieving the delicate balance between the conflicting values of privacy and access to healthcare that is highlighted in the scenario (Ansarian & Baharlouei, 2023). A3: Justification of Practices or Policies I choose policy 4.3.3 from Domain 3, which is "Honest and accurate information and education of the public" (American Public Health Association, n.d.). This policy calls for 4 organizational honesty when it comes to the strengths and weaknesses of the offered services, along with their limitations and risks. The policy will deal with the ethical issue by restoring Professionalism and Trust following data breaches and giving patients the ability to make well informed decisions regarding telemedicine. This policy assists in resolving the issue between Human Rights and Civil Liberties (privacy) and Health, Justice, and Equity (access) by communicating openly about security measures and possible risks (Solimini et al., 2021). I chose practice 4.7.3 from Domain 7 which states that health promotion should be based on respect for the dignity and ability of people rather than on any tactics of stigmatization (American Public Health Association, n.d.). This policy highlights the importance of dignity and equality of resources access while providing protection for vulnerable groups. This policy is important because it helps the organization develop telemedicine services that are secure without posing any threat to patient confidentiality. It combines privacy protection with sustained access and promotes Health Justice and Equity without infringing on personal rights (Ansarian & Baharlouei, 2023).
