Health history

A.P. is a 41-year-old female of Indian descent. She is 64 inches and weighs 180 pounds.

She has a past medical history of abdominal pain, hypoalbuminemia, renal insufficiency

syndrome, anemia, eye hemorrhage, beta blocker overdose, hyperkalemia, and diabetes. Her

past surgical history includes cesarean section. Patient is allergic to sulfonamide antibiotics.

She is able to ambulate without assistance, and is on 1800 ADA/calorie diet controlled diet.

Patient also is a vegetarian. She came in to the emergency department complaining of

vomiting, nausea, and abdominal pain. She was then transferred to the diabetic/ metabolic

unit due to her fluctuations in blood sugar. Upon my initial assessment, her blood glucose

sugar was 53.

Immediate action had to be taken and orange juice was given to the patient as well as

her breakfast. After, 15 minutes her blood sugar was checked again and it had decreased to

45. Increasing her blood sugar was the main priority throughout my clinical shift. At first, 10

% dextrose had to be administered, but the nurses were unable to IV push due to her

ineffective peripheral line. Several attempts were made to start a new peripheral line but all

failed. The surgical team came and assessed the patient and suggested that she needed a

peripherally inserted central catheter (PICC). In the meantime, 1mg of glucagon was injected

intramuscularly. The glucagon helped increase her blood sugar to 86. Patient also was

retaining a lot of fluid and appeared bloated. She had generalized edema +1. Her usual weight

is 140 pounds, but because of her fluid retention her weight increased to 180 pounds.

Temperature: 97.5 F

Blood pressure: 143/64
Pulse and heart rate: 81 beats per minute
