Key Labs & Rules
Ferritin ↓ first in iron deficiency anemia (IDA)
Low iron + ↑ TIBC = IDA
Low iron + ↓ TIBC = Anemia of Chronic Disease (ACD)
Normal RDW + very low MCV = Thalassemia
Men & postmenopausal women w/ IDA → GI workup mandatory
Reticulocytes
Retic index < 2% → ↓ RBC production (marrow problem)
Retic index > 2% → hemolysis or blood loss
Macrocytic Anemia
B12 deficiency
o ↑ methylmalonic acid
o ↑ homocysteine
o Neurologic symptoms (may be irreversible)
Folate deficiency
o ↑ homocysteine only

o NO neuro symptoms
Always rule out B12 before treating folate
Hemolysis Labs
↑ LDH
↑ indirect bilirubin
↓ haptoglobin
↑ reticulocytes
