or practitioner at a distant site through telecommunications equipment that connects the patient

Telehealth adoption allows health care providers to bridge the gap in care by increasing
continuity of care, reducing patient travel burden, especially in rural and other underserved
populations, which ultimately helps health systems and physician practices focus more on
chronic disease management, enhancing patient wellness, improving efficiency, providing
higher quality of care, and increasing patient satisfaction. Some studies have shown how
interventions involving telehealth implementation improved HbA1c levels, treatment adherence,
and self- efficacy in diabetic patients. Implementing telehealth within this organization provides
the optimal alternative solution to traditional in-person care delivery for the underserved patients
in our community who live further than 45 minutes away from our facilities and suffer from
chronic health conditions such as hypertension and diabetes.

The Chief Financial Officer (CFO) is the senior executive responsible for overseeing an organization's
financial operations. As a key advisor, the CFO analyzes the financial aspects of this project from the
beginning which entails knowing how much it will cost, how it will be financed, the financial
impact of the project implementation phase, and evaluate budget and Return On Investment
(ROI) against project objectives. The CFO will also explore diverse funding opportunities
through government grants, sponsorships, community support, vendor discounts, and internal
budget adjustments.
The Information Systems Technician (IST), or the Information Systems Architect (ISA), will assume the
pivotal role for this implementation, by applying a comprehensive approach to system design,
meticulously planning the architecture and infrastructure to support the project's objectives,
implementing system integration, implementing robust measures to safeguard data and systems,
strategic vendor selection, conducting thorough evaluations of emerging technologies, and contributing
to meticulous budget planning

The Systems Administrator (SA) plays a vital role in maintaining the operational integrity of an
organization's IT infrastructure by ensuring the smooth functioning of both software and
hardware, proactively monitoring system performance, and swiftly resolving technical issues to
guarantee stability and security.
The Informatics Nurse Specialist-Project Manager (PM) is a crucial leader who oversees
healthcare technology projects from start to finish. They are responsible for team formation,
resource allocation, ensuring the project meet deadlines, budgets, and quality standards,
managing risks; as well as effectively communicating project progression with the
multidisciplinary team and stakeholders.

PLANNING:
The planning phase of the Software Development Life Cycle (SDLC) is crucial for defining a
telehealth project's scope, objectives, and direction. This involves conducting feasibility studies,
ensuring EHR infrastructure alignment and regulatory compliance, and gathering stakeholder
input to identify system needs and workflows. Key activities include developing timelines,
budgets, and risk mitigation plans, culminating in final approvals for project progression.

1. Defining the Project Scope: This step places emphasis on thorough planning and
market research. The Chief Information Officer (CIO) and Nurse Informaticist- Project
Manager (PM) collaborate to establish and define the needed core functionalities of the
telehealth application, define clear project objectives, determine its size, and set
parameters that align with the organization's strategic goals. They will perform a
