Chief Complaint (CC)
"My child had a high fever for a few days and now has a rash."
History of Present Illness (HPI)
Kecia Young is a previously healthy infant who presents with a history of
high fever up to 103–104°F (39.4–40°C) lasting approximately 3–4 days,
which resolved abruptly. Within 24 hours of fever resolution, the parent
noticed the onset of a pink, maculopapular rash that began on the trunk
and spread to the neck and extremities. The child remained active and
,playful after the fever resolved. No history of cough, rhinorrhea,
conjunctivitis, vomiting, or diarrhea. Appetite slightly decreased during
febrile period but now improving. No known sick contacts. Immunizations
are up to date for age.
No history of seizures, although parent expressed concern due to the high
fever. No recent travel or exposure to new foods, medications, or
detergents.
Past Medical History (PMH)
• No chronic illnesses
• No previous hospitalizations
Birth History
• Full-term vaginal delivery
• No complications
Medications
• Acetaminophen PRN during febrile period
Allergies
• No known drug allergies (NKDA)
Family History (FH)
• Noncontributory
• No history of immunodeficiency or seizure disorders
Social History (SH)
, • Lives at home with parents
• Not in daycare
• No tobacco exposure
Review of Systems (ROS)
• General: Recent fever, now resolved
• Skin: Pink rash on trunk and extremities, non-pruritic
• HEENT: No congestion, no conjunctivitis
• Respiratory: No cough or SOB
• GI: No vomiting or diarrhea
• Neuro: Alert, playful, no seizures
All other systems reviewed and negative.
