Age: 25 year old

History of Present Illness (HPI)
The patient is a 25-year-old female who presents with complaints of
loose stools for approximately 4–5 days. She reports passing watery to
semi-formed stools 4–6 times per day. Symptoms began abruptly and
have been persistent. She describes associated mild lower abdominal
cramping that improves after bowel movements. She denies blood or
mucus in the stool, fever, chills, nausea, or vomiting. She reports no
recent travel outside the country, no known sick contacts, and no recent
antibiotic use. She admits to eating food from a local restaurant shortly
before symptom onset. She denies weight loss, nocturnal diarrhea, or
symptoms waking her from sleep. Hydration has been maintained,
though she feels mildly fatigued.
Past Medical History (PMH)
• No chronic medical conditions reported
• No prior gastrointestinal disorders
Past Surgical History (PSH)
• Denies any past surgeries
Medications
• Oral contraceptive pill daily
