Introduction
Referral by the GP
 Mr PH is a 24 year old male who experiences lower back and knee pain.
 After expressing his concerns to his girlfriend, she advised him to visit a General Practitioner(GP).
 Since lower back pain is considered as the most common condition worldwide, a general practitioner is the
most ideal choice to refer to. ap2
 Furthermore, with the help of the x-ray photos the GP has diagnosed that PH had no specific pathology
causing his problems. Therefore, he advised him to rest for a short period of time and minimise any
excessive activities.
 However, if the problems continue to exist then he should visit a physiotherapist without first consulting
with the GP.
 The physician has also emphasised how important the daily nutrition and physical activity is in order for PH
to become healthier and progress with his treatment.
Main health risks
 As we know, Mr. PH has extra weight as he is 20 kilograms above his height which classified him as obese in
the BMI scale.
 Furthermore, he is a smoker which increases his chances of suffering lung cancer.
 Additionally, he is going to take a promotion in his job and because of the lower back and knee pains he
thinks that he will lose the chance of being promoted. So, another risk factor is stress and anxiety.
 Of course, we can’t skip the situation that he is no longer an athlete which illustrates that his life is
sedentary.

Disorders and limitations
 His participation to every day tasks will be eliminated.
 P.H. impairments will cause him to be depended on others for transportation, taking care , cleaning etc.
and he might not even be able to socialize with others.
 His job requires him to lift heavy objects so he could be hired and loose his promotion.
 Continuing to put pressure on his back and his knee might result to an increase of the pain
 Being obese, it can lead to serious health issues like cardiovascular diseases or orthostatic problems.
 Moreover, as a smoker he can experience impairments in his respiratory system leading to lung cancer.
The medical history of P.H.
He is a 24 years old young man born in Servia and lives in Netherlands since he was 12 years old. The first sign of his
problem started 7 years ago when he stopped playing football.
