Remote Collaboration and Evidence-Based Care Good afternoon everyone and welcome to my Remote Collaboration and Evidence-Based Care Video for NURS4030: Making Evidence-Based Decisions. My name is Marissa Palko and During this video, I plan to:  Propose an evidence-based care plan to improve the safety and outcomes for a patient.  Explain the ways in which an evidence-based practice model was used to help develop the care plan.  Reflect on which evidence was most relevant and useful when making decisions regarding the care plan.  Identify benefits and strategies to mitigate the challenges of interdisciplinary collaboration to plan care withing the context of a remote team.  I will make mention of the Vila Health Scenario which mentions a child recently diagnosed with cystic fibrosis. Providing care for patients who are unable to come into the physician’s office for a follow-up appointment after being hospitalized can be a difficult task.
