P6 – explain why meeting the needs of the individuals requires the involvement of different
agencies
Multiple agencies are needed to be involved with each other so they can provide specific
care to an individual’s care. Each agency will bring different information with them and
expertise knowledge that will help improve the standards of care that service users receive.
Clinical commissioning groups
The clinical commissioning group a is clinically led body of people who are responsible for
planning for and commissioning health care services that are in their local area. Their role is
to get the best outcomes for certain service users. They asses the needs of the local people,
they decide on priorities and strategies for health and social care, and they then buy
services on the behalf of the population. They need to consistently respond and to adapt to
circumstances. They are responsible for the health of the population, and it is measured by
how they can improve the outcomes.
Organisations commissioning social care services
Local authorities will commission social services to become personalised
and community-based support that promotes health and well-being for
service users. The care act presents local authorities with challenges about
commissioning advocacy services. They make sure there is enough provision
that meet demands for now and in the future and they make sure that
advocacy services are aligned and that they are developed with other
developments in revies and assessments. Local authorities will work closely
with other organisations like the NHS. They will use frameworks as a
guidance like the adult social care outcomes guidance, public health
outcomes framework and making it real statements.
, Health and wellbeing boards
Health and wellbeing boards were established to be a forum where key leaders from the
local health and care system can work together so they can work together to improve the
health and wellbeing of their local area.
Role of assessment frameworks
Assessment and eligibility frameworks are important for integrating health and social care.
They should lead to things like less inequalities in service provision and greater clarities and
consistency and integration in services. It also means there should be a greater focus on
prevention by considering service users care and support needs.
Education, health and care plans
An education health care plan is a document which replaces statements of SEN and learning
difficulties assessments for young people and children who have special educational needs.
It can only be issues after the individual has gone through the EHC needs assessment.
Meeting the needs of case studies by different agencies
Different teams will work together to ensure his needs are met. He has occasional seizures
that are mostly controlled by seizures so teams will need to ensure that the length if the
seizure is monitored and that an ambulance should be called if it is over five minutes, they
will make sure that anything that could harm him is out of the way and that he is placed in
the recovery position. He is unable to collect his medication without support so they will
ensure that his medication is picked up on the first Thursday of each month. And he is
unsteady when transferring weight from a bed or aa chair and he will not use a lifeline so
they will encourage the use of equipment and check for any possible tripping hazards, they
will also discuss a lifeline with him when its possible. This will all ensure that a safe
environment is kept. R is not able to prepare his own meals and is not eating very much as
he says he has no appetite the team will establish that people need to prepare meals for
him four times a day and he should be encouraged to eat and drink regularly and he should
be offered snacks, thus prioritises his health.
Jim had several falls at home, and he has a lifeline alarm that he can press if he needs help,
the team should let people know that he should be encouraged to check his lifeline often to
ensure that it is still working. He likes to be independent, but he attempts to do everything
for himself, for example the care staff should help him to get up and get dressed he’s
normally up and dressed when they arrive, to ensure his safety teams will make sure that
there are no hazards to Jim like a rug when they are arriving and leaving. Jim has to take
frusemide twice a day and he takes warfarin once a day, staff will ensure that his daily
medication is taken, they will dispense weekly medication into medical wallets on Sundays
and they will ensure that the medication is stored safely where it is only accessible to staff
who need to use it, medication will be collected from the pharmacy and will be re-ordered if
a repeat prescription is needed on the last Friday of every month. This ensures a safe
environment for Jim.
