“My right eye has been red and irritated since yesterday, and it feels
uncomfortable.”
History of Present Illness (HPI) Clinician:
“When did the eye problem start?” Patient:
“It started about 24 hours ago.” Clinician:
“Was the onset sudden or gradual?” Patient:
“It came on pretty suddenly.” Clinician:
“Is the problem in one eye or both?” Patient:
“Just my right eye.”
, Clinician:
“Can you describe what you’re feeling?” Patient:
“It feels itchy and gritty, like something is in my eye.” Clinician:
“Do you have any eye pain?” Patient:
“It’s uncomfortable, but not really painful.” Clinician:
“Any change in vision or blurriness?” Patient:
