16-Year-Old with Symmetric Hand Pain
Diagnosis: Polyarticular Juvenile Idiopathic Arthritis (RF-
Positive)
Chief Complaint (CC)
“My hands hurt on both sides, especially in the morning.”
History of Present Illness (HPI)
The patient is a 16-year-old adolescent who presents
with bilateral hand pain and stiffness that has
progressively worsened over the past 4 months. The pain
is symmetric, involving the metacarpophalangeal (MCP)
and proximal interphalangeal (PIP) joints of both hands.
The patient reports morning stiffness lasting more than
one hour, which improves gradually with activity
throughout the day.
