"My hand has been hurting."




HISTORY OF PRESENT ILLNESS (HPI)
The patient is a 16-year-old male who presents with hand pain. The pain is localized to the hand
and began after routine activities. He denies any major trauma. The pain is worsened with
movement and improves with rest. He denies numbness, tingling, or weakness. There is no
associated fever or systemic symptoms. The pain prompted evaluation in the outpatient clinic.




REVIEW OF SYSTEMS (ROS)
 General: Denies fever, chills, weight loss
 HEENT: Denies headache or vision changes
 Cardiovascular: Denies chest pain or palpitations
 Respiratory: Denies shortness of breath
 Gastrointestinal: Denies nausea, vomiting, abdominal pain
 Genitourinary: Denies urinary symptoms
 Musculoskeletal: Positive for hand pain
 Neurological: Denies numbness, tingling, or weakness
 Skin: Denies rash or open wounds
