In this section of my report, my care manager has asked me to attend
multi-disciplinary meetings for Gina and Martin. The care manager has asked me
to let him know how different healthcare professionals work together to meet
Gina’s and Martin’s needs and how multi agency and multidisciplinary get
involved with a justification. While maintaining confidentiality and managing
information.

Meeting the needs of the individuals requires the involvement of different
agencies. This is because, one major problem that the health and social care
system has faced over the past years is that there has been a lack of coordination
across all types of services and integrated care. For example, a service user who
has been in contact with different agencies, would have to share the same story
about their health issues. This can be frustrating and devastating for the service
user as different agencies display a lack of communication and lack of logging in
the service’s user story such as emails or transcripts. This is where a
multidisciplinary team and multi agency comes in place. In a health and social
care setting, a multi disciplinary team refers to the involvement of different
healthcare professionals who come from different organisations to decide which
treatment would be the best for a service user. There are a number of different
healthcare professionals who may be involved in a multidisciplinary team: GP,
Nurse, Doctor, Social worker, Occupational therapists, Dietitian, and Clinical
psychologists. A multi agency refers to healthcare professionals across different
agencies being collaborative to achieve goals and objectives. For example, GPs,
social workers, nurses would together make decisions regarding giving treatment
to service users who may have asthma attacks. There are a number of different
healthcare professionals who may be involved in a multi agency: Social workers,
physiotherapists, community nurses, occupational therapists, mental health
specialists and local GPs. Multi agency is needed because there can be problems
when coming up with solutions, procedures and decisions. For example, if a nurse
and a doctor have a lack of coordination then that means they could get the
wrong treatment. However, multi agency allows service users with different needs
to have support from different expertise.



In the case of Martin, the multi agencies and multi disciplinary team that will
work with Martin are GP (general practitioner), Practice nurse and asthma nurse,
Pharmacist, School nurse, Respiratory specialist (hospital consultant), Respiratory
physiologist, Speech and Language Therapist, Educational psychologist and
occupational therapists. These professionals and the different agencies involved
to meet his needs would create a treatment plan so that they can treat his
asthma attacks and be able to carry out EHC assessments as he needs extra
support in his education.

In the case of Gina, the multi-agencies and multidisciplinary team that will work
with Gina are Psychiatrist, Neurologist, School nurse, Occupational therapists (for
Ray), Young carer, Clinical psychologists, Social worker and GP. These
professionals and the different agencies involved to meet her needs would allow
her to receive holistic treatment which means that her wellbeing will improve.



Ultimately, meeting the needs of individuals requires the involvement of different
agencies because it “ensures that the diverse care and support needs of
individuals are met.” (Study rocket, 2024). As a result of this, service user’s lifestyles
will improve but so would their quality of life.



In terms of health and social care terminology, a commission refers to the
“process of assessing needs, planning and prioritising, purchasing and monitoring
health services, to get the best health outcomes.” (NHS England, no date).



Healthcare organisations are responsible for commissioning in healthcare
services across the UK. The following health and social care services are listed
below: Clinical Commissioning Groups England (CCGs), Local Health Boards in
Wales, and Health and Social Care Board in Northern Ireland.



CCGs

Clinical Commissioning Groups or also known as CCGs are “clinically-led statutory
NHS bodies responsible for the planning and commissioning of health care
services for their local area.” (NHS UK, 2021). The aims of the CCGs were to decide
how to best carry out the needs of a population such as the UK and to decide
which services can be delivered to service users depending on the flexibility of
providers such as hospitals, clinics, etc. As the CCGs were involved due to the
adaptation of a population’s circumstances, the ICBs were created to improve a
population’s health and wellbeing. Healthcare professionals that are members of
the CCGs are: GP practitioners, Nurse, Clinicians, Consultant and other
professionals such as Doctor. Not to mention, one of their key roles is to provide
rehabilitative care. This type of care refers to retaining good health and wellbeing
through the use of therapy and education. For example, a service user that has
lost the ability to speak would need to go to speech and language therapy to
regain their speaking abilities.

Local Health Boards in Wales

There are 7 local health boards that are part of the NHS in Wales and operate
alongside three NHS trusts which are the Welsh Ambulance Service NHS Trust,
Public Health Wales NHS Trust and Velindre University NHS Trust. The local health
boards are responsible to provide services near people’s homes such as self care
services and they plan long term to deliver public health by providing primary
care, hospital and community services. The members that would be involved in
the local health boards in Wales would be GPs, Dentists, Pharmacists, Opticians,
Nurses, and Optometrists. For example, optometrists would do examinations to
ensure that a service user or a patient’s eye health such as vision is healthy from
certain diseases such as diabetes or things like high blood pressure. If they detect
that a person’s eye health needs to be improved they would prescribe you with
prescribed glasses or contact lenses.



Health and Social Care Board in Northern Ireland

The health and social care board in Northern Ireland was formed in April 2009
because they wanted to modernise how health and social care services were
managed and wanted to make their services more effective by making sure that
they use the necessary resources and being adequately productive. Ultimately,
the health and social care board (a non profit statutory body) in NI (Northern
Ireland) is made up of five local commissioning groups and five health and social
care trusts. Their main role is to commission for the population in Northern
Ireland and manage performance based on the services delivered. The members
that are part of the health and social care board in Northern Ireland are chair and
non-executive members.



The healthcare organisations such as the ones mentioned above also do
partnership with some social care organisations as well such as NHS partners,
using guidance such as the Adult Social Care Outcomes Framework, Making it
Real statements and the Public Health Outcomes Framework. In addition, there
are other organisations such as Health and Wellbeing Boards and Integrated Care
Systems (ICS).



Health and Wellbeing Boards

Health and wellbeing boards are statutory bodies which were introduced by the
legislation (The Health and Social Care Act 2022) to be able to serve as a forum for
all health and social care leaders who came together. Their main role is to improve
the health and wellbeing of a local population and reduce health inequalities. This
could mean the access to a certain care or the quality of health and social care
services. The aim of the HWBs was to encourage more partnership between
different statutory bodies and organisations such as joint commissioning. For
example, appropriately allocating funds and investing on the ‘service
development fund’ which comes from the NHS to improve their services.
