Four declaration forms are required in this section: 1. Language Declaration Form o Covers Home Language or First Additional Language. o Confirms that the student has prepared and taught at least one complete lesson in the chosen language subject. o Signed by both the mentor and the principal. 2. First Chosen Subject Declaration Form o Confirms teaching of at least one full lesson in the first selected non-language subject. o Signatures of mentor and principal required. 3. Second Chosen Subject Declaration Form o Confirms delivery of a full lesson in the second chosen subject. o Requires all necessary signatures and school stamp. 4. Third Chosen Subject Declaration Form o Confirms that the student taught a lesson in the third selected subject area. o All signatures and stamp required. Each declaration form contributes to the portfolio mark allocation.
