Introduction
The safety and well-being of employees and building occupants remain a critical focus
of occupational health and safety (OHS) management in South Africa and globally.
The Bank of Lisbon Building Fire, which occurred in 2018 in Johannesburg, tragically
exposed the severe consequences of non-compliance with basic fire safety
regulations and the absence of effective emergency preparedness strategies in public-
sector buildings. The incident, which led to the deaths of three firefighters and injuries
to several others, highlighted systemic failures in safety management, fire detection,
emergency response, and evacuation procedures. This assignment explores the
multiple factors that contributed to the fire's outbreak and rapid spread, identifies the
critical safety violations, and evaluates the evacuation challenges experienced by both
firefighters and occupants. Drawing from relevant occupational health and safety
principles, fire safety regulations, and lessons learned from this case study, the
discussion further presents strategic recommendations to improve fire prevention,
infrastructure design, and safety planning in multi-storey office buildings. The insights
gained from this tragedy highlight the urgent need for integrated fire safety systems,
strong regulatory enforcement, and enhanced employee awareness to ensure
organisational resilience and safeguard human life.

What was the primary cause of the Bank of Lisbon Building Fire?
The fire was initiated by an electrical fault originating in the electrical supply room
located on one of the upper storeys of the building (Nkuna, 2024: 77). Electrical failures
such as this are particularly hazardous in large, ageing buildings where systems may
be poorly maintained or outdated. These faults can generate significant heat and ignite
