Healthcare Problem Analysis Problem
There is a major issue with hospital readmissions because many of these cases can be prevented
in the first place. Health care costs can increase because of these preventable readmissions,
which also compromise the health of patients. The 30-day readmission rate has become an
important indicator of healthcare quality in the U.S. Healthcare facilities are facing financial
strains due to increased patient expectations and penalties imposed by the Centers for Medicaid
& Medicare Services (CMS) Hospital Readmissions Reduction Programs (HRRPs) (I. Kum
Ghabowen et al., 2024). It is reported that adult hospital readmission rates are still challenging,
contributing approximately $41 billion to U.S. healthcare costs each year.
Measurements and Assessment of the Problem
It is important to find specific performance metrics prior to assessing the severity of preventable
readmissions. As well as helping diagnose current issues, these metrics can also track progress in
enhancement initiatives. The main performance standards include:
30 Day Hospital Readmission Rates: Health insurance policy that tracks and measures
readmissions within 30 days of discharge from a hospital (CMS, 2023).
, Patient Fulfillment Results Associated to Discharge Planning: By utilizing HCAHPS
surveys, hospitals can measure patient experiences standardized, which allows them to compare
facilities objectively. By finding areas where patients are dissatisfied, hospitals are encouraged to
improve quality of care, and consumers can make informed decisions about where to receive
healthcare based on publicly available patient feedback (Centers for Medicare & Medicaid
Services, 2023).
Completion Rates for Follow-Up Appointments: Patients who comes to check-up visits within
7 days of dismissal are less likely to be readmitted. By watching these rates, we can assess
continuity of care initiatives' efficacy.
Reconciliation of medications at discharge: Reconciliation of medication orders make mention
of the task of contrasting a patient's medication sequence with his or her medication orders. By
reconciling medications, omissions, duplications, wrong dosages, or drug interactions can be
prevented (Medication Reconciliation | Electronic Health Record (EHR), n.d.).
Using these metrics, healthcare systems can find and label parts giving to avoidable
readmissions, resulting in improved clinical results and cost savings.
Centering on these domains will help healthcare systems appreciate and direct the elements
