Client Name: JB
Date: 02/23/2025
Counselor: Dr. Yanez DSW, LCSW
Session Number: 1
Presenting Problem: Husband is an alcoholic and she is depressed in trying to keep her
relationship together
D: DATA
showed up for her session. The client expressed worries about her husband's alcoholism, her own
sadness, and her attempts to keep their marriage intact. She explains that she and her husband
have been together for about six years, during which time he has been drinking. She also claims
that it has gotten worse over the last six months and that he has been verbally and physically
violent. She claims that the verbal abuse includes him informing her which man she is seeing,
accusing her of cheating on him, and claiming that she is tardy to work. And that she is
unattractive and lazy, and that he slammed her against the wall because of the physical abuse.
She said that he had not struck or slapped her and that she had voiced her concern of possible
bodily harm. However, the customer stated that she fears he could harm her. The client expresses
her sadness at not being content in the relationship at the moment. She used to be joyful, but now
she is not, and she is disheartened. She expresses her desire for him to give up alcohol. The client
claims that she feels unworthy and asks what is wrong with her that would cause him to treat her
the way he does today. She also claims that she believes he is cheating on her, which is why he is
accusing her of cheating and that she is unworthy of him. The client claims that she has trouble
sleeping, is undernourished, has little appetite, has been exercising excessively, and is not feeling
well as a result of her poor diet. She claims that she doesn't spend as much time with her family
as she formerly did and that she never goes out with friends because he believes that she is doing
something wrong if she does. that she was once more joyous and content. She claims that her
humor and jokes have vanished and that she is now inherently more serious.

A: Assessment

Her husband's drunkenness and violent behavior are causing the client great emotional suffering.
Evidence of maladaptive coping techniques, such as excessive exercise, insufficient eating, and
feeling ill while not eating, suggests that her current home situation is directly linked to her
depression and physical systems. She feels secure in her marriage and has a high sense of
self-worth. Her anxiety is having a significant impact on how she functions on a daily basis,
particularly in her interactions with her daughter and as a parent. Her psychological discomfort
seems to be primarily caused by the maltreatment. Additionally, her physical systems—such as
digestive problems and sleep disturbances—are making the total effect on her health worse.
