AYM1 SERVICE PLAN BRIEF Service Plan Brief for Diabetes Education Clinic Service Idea The service idea that I would like to implement in my community would be a stand-alone diabetes education clinic. While researching the community that I live in, I learned that every diabetic education clinic in the area is all located inside of hospitals. Working in the hospital myself I also know that people in my community do not like to frequent the hospitals, therefore not receiving the services that they need daily to manage their health. The purpose of the community-based education is to prepare people with diabetes to have the skills and confidence to manage their disease daily and to manage its impact on life roles and emotions in an environment which is more convenient and familiar to the target population (Association of Diabetes Care &Education Specialist, 2017). The stand-alone clinic will be centrally located in an area which is easily accessible to all members of the community. The community will have the option to utilize the community-based program rather than feeling obligated to attend a program in the neighboring hospitals. Managing diabetes is a team effort that involves health care providers, diabetes self-management education, family and friends and community support Centers for Disease Control and Prevention, n.d.). The goal of providing this service is to have community involvement in treating and managing diabetes in our own neighborhood, to help reduce the risk of other health complications. Diabetes education is important in the management of diabetes because this is a medical condition where management changes over time. A regimen that has worked in the past may no longer be effective for management in the future. Also having a community center for education will give those who need access to water and healthy eating habits the tools that they need to succeed in managing their own care at home. A healthy diet, regular physical activity, PAGE 2 AYM1 SERVICE PLAN BRIEF maintaining a normal body weight and avoiding tobacco are ways to prevent or delay the onset of type 2 diabetes (World Health Organization, 2020). The education center is not solely focused on changing habits of those with diabetes, but also preventative measures for those who are at risk due to family history or health risk factors. By having the community education center will give the population help in gaining access to educators with experience in diabetes education. Educators can be any licensed professional like Registered nurses, Doctors, Pharmacist, Registered Dieticians and so on. Educators are in the field to empower their patients to make life changes that will help to manage their diabetes. The goal of the educator is to guide and teach their patients so that they can understand their medical condition and how it affects their personal life, but also to work with them on different ways to improve their health and change current behavior. The education center will give the community access to cost-saving interventions like blood glucose monitoring, blood pressure control, foot care, exercise regimens, and healthy food options.
