Took a nurses training course Person-centred care is care that is less focused on the pathology and disease and more focused on the social determinants of health. It is showing humanity in medicine by providing care for one patient at a time, putting the needs and preferences of the patient first and allowing the patient and their families to be an active participate in formulating their own plan of care. Many practitioners practice population-based care and place too much focus on the diagnosis itself while paying no attention to the patient. In person or patient-centred care, the patient’s beliefs and values are taken into account and they provided with the respect, education and support to make their own decisions in how they are treated. As leaders in healthcare, advanced practice nurses (APN) have the opportunity to do away with provider-driven care and implement a person-centred care approach while applying principles of holistic nursing, cultural humility and self-reflection. In my future role as a nurse practitioner (NP) I will provide multidimensional care tailored to meet the specific needs of each and every patient and apply the principles of holistic nursing, cultural humility, and self-reflection
