A1. Problem or Issue To come up with a problem that needs resolved within our company, I communicated with my nurse manager, nurse director, office staff, and other nurses/CNAs. The issue that I decided upon is the lack of effective communication among all staff members. The proposal I am recommending is providing meetings for interaction as well as education. I also am providing a method (the electronic nurse’s note) that will help lead to improved and effective communication among staff members. A1a. Explanation of Problem or Issue I work for a home health company that provides skilled nursing for children with special needs. The nurses and CNA’s in our company work with different patients in different environments including the home and school setting. It is a one on one patient/nurse based care situation. Because of this, as a healthcare staff we rarely interact with each other. We do have quarterly training and a few parties where we get to interact, but they are not frequent occasions. This means that most communication amongst the staff is done through communication notes and charting. Effective communication within the staff, patients, and their families is critical to providing the best care possible. Lack of effective communication can lead to healthcare workers not receiving the proper information needed to provide quality, safe care for the patients. I have seen a lack of communication in healthcare members not properly documenting in their charting or not being detailed enough with information. It has also been seen that a team member may not chart or document something because they do not feel it is crucial, when in fact it may be pertinent information that will help provide quality care. Because of the limited amount of physical interactions between health care workers, using other forms of effective communication is definitely an important component of quality care for the patients I work with. ......................................CONTINUED.......................
