Sara Emalee Pate Grand Canyon University – NRS493 December 4, 2020 Capstone Change Project Evaluation Plan Evaluating the outcomes and data of a change project is essential to see its effectiveness and to see if any modifications must be made to the change project. After discussing with my preceptor on how to rigorously evaluate my change project, we concluded that the best way to evaluate the project would be to gather data on catheter-associated urinary tract infection incidence by using chart audits, informatics, readmissions, and patient diagnosis during
hospital stays. Our informatics team in our organization currently already collects data on catheter-associated urinary tract infection incidence within the organization every 6 months and separates it by department. This helps to identify or narrow down the issues across the organization. So, every 6 months the directors of every unit obtain this information from our informatics team and reveal the data to the staff. To evaluate my change project, we would utilize this same approach by the informatics team and compare the incidence rates of catheter-associated urinary tract infection before implementation and after implementation of the change project. Now to evaluate that the nurse driven protocol is being utilized by nurses, we would have our same informatics team evaluate whether nurses are charting the usage of the protocol on patients that have indwelling catheters by doing chart audits on every patient that has an order for insertion of a urinary catheter. This allows for us to evaluate the effectiveness of the nurse-driven protocol and if it is being used correctly, if the catheter-associated urinary tract infection incidence remains the same, but it is notable that our nurses are not utilizing the nurse driven protocol correctly, then we would need to reinforce the proper usage of the nurse driven This study source was downloaded by 100000770861734 from CourseHero.com on 05-03-2021 09:30:52 GMT -05:00
