HISTORY & PHYSICAL:
A 25-year-old gravida 1 para 0 female client presents to the emergency department reporting heavy
vaginal bleeding and feeling faint. She states that she "passed out" in the shower and has a small head
laceration that is not bleeding. She reports soaking through at least one pad/hour for the past 4 days.
She states that her periods are irregular and that they are often very heavy. She also has abdominal
tenderness and at times, severe abdominal pain that started with her last period and seems to be
getting worse.
Medical History:
Asthma: onset age 13
Tonsillectomy: age 14
Spontaneous abortion: age 23 years at 9 weeks' gestation
Her vital signs on arrival are consistent with hypovolemia. Blood pressure is 95/50, pulse 108,
respiratory rate 20, and temperature 99.0 °F. The healthcare provider orders an abdominal and vaginal
ultrasound and laboratory work.
