month.”
History of Present Illness (HPI)
Lucretia D. is a 56-year-old female presenting with a 1-month history of worsening low back
pain radiating into the left leg. Pain is described as sharp, burning, 6/10 at baseline and 8/10
with prolonged sitting. Pain radiates from the lower lumbar region to the posterior thigh and
lateral calf.
Symptoms worsen with:
• Sitting > 20–30 minutes
• Bending forward
• Lifting objects
Symptoms improve with:
• Rest
• Heat application
• Walking short distances
She reports occasional numbness and tingling in the left foot. No bowel/bladder incontinence.
No saddle anesthesia. No recent trauma.
Denies fevers, weight loss, IV drug use, or cancer history.
