Chief Complaint: "I am always faced with an extensive feeling of worry, fear, and anxiety to
the extent that my daily activities are affected. I have been struggling with anxiety and
depression since the summer of 2021."

History of Present Illness: P.K is a 32-year-old woman of African American descent. P.K.
presents extensive worry, fear, anxiety, and depressive symptoms. P.K. presents anxious but
cooperative, often playing with her hair during the interview. She reports she has been struggling
with worsening anxiety and depressive symptoms since the summer of 2021.
She relates that she has always been an anxious person and has dealt with some
depression throughout her life but has been able to control it most of the time and has never felt
the need to seek treatment. However, since the summer of 2021, symptoms have increased. She
notes that during the summer of 2021, she witnessed her neighbors getting robbed, and since that
point, her anxiety has "skyrocketed." P.K. describes feeling fearful all the time, nervous, has
been having panic attacks, and depressed mood. She states, “I felt like my life is crumbling.”
She states that in the past few months’ symptoms have worsened and have increased her
depressed mood and anxiety. The patient states that she constantly worries that something
terrible is going to happen. P.K. also reports losing interest in doing things that she once used to
love to do, such as running, hiking, spending time with friends and family, or baking; she
sometimes feels hopeless, has difficulty focusing, and tries to go out with friends or family but
ends up focusing on if somebody is going to break into her house or starts to drive somewhere
and turns around and goes back to recheck the locks. She said that people around her have
described her mood as not the same and quiet. She reports having panic attacks that can last for a
few minutes to longer. She also finds herself checking the locks on the doors and windows,
which interferes with her daily life. The patient reports interrupted sleep.
The patient has had issues of being worried and fearful to the extent that it has affected her
everyday life. The patient has never been officially diagnosed with mental health issues. The
