History of Present Illness (HPI)
Mr. J.D., a 65-year-old man, presents to the outpatient clinic complaining of
fatigue and frequent nighttime urination (nocturia) that started about 3
months ago. He reports waking up 3–4 times per night to urinate and feeling
increasingly thirsty throughout the day. He also notes increased appetite but
has unintentionally lost about 5 kg in the past two months.
He denies pain or burning during urination, fever, or flank pain. He reports mild
blurred vision and occasional tingling in his feet.
No previous diagnosis of diabetes or prostate problems. He has a sedentary
lifestyle and eats mostly processed foods.
