Reason for Encounter: Cough and fever
S: Subjective
Chief Complaint:
“Esteban has had a bad cough for three days, and last
night he had a fever,” reports his mother.
History of Present Illness (HPI):
Esteban Soto is a 7-year-old boy brought to clinic by his
mother due to cough and fever. The cough began three
days ago, is productive of yellow sputum, and has been
progressively worsening. His fever peaked at 38.9°C
(102°F) last night, partially relieved with acetaminophen.
He reports mild shortness of breath when active and chest
discomfort when coughing. No vomiting or diarrhea.
Appetite is decreased but still tolerating fluids.
Associated Symptoms:
• Positive: fatigue, decreased activity, mild wheezing
at night.
• Negative: ear pain, sore throat, abdominal pain, rash,
hematuria.
