NCLEX RN 121L-B Ostomies 2023
-
- Small intestine: nutrition and minerals
- Large intestine: water and electrolytes
- Ostomy: opening surgically created in the abdominal wall for the elimination of feces or
urine
- Stoma: the surgical opening in the skin
- The end of the bowel that is brought up to the surface via the abdomen


Appearance of a Healthy Stoma:
- Round/oval shape
- Red/pink in color
- Protruding or 2 cm from abdominal wall
- May be flush against the skin
- Moist (similar to mouth’s mucus membranes)
Abnormal Stoma (Must Report):
- Pale, dusky, brown, or black stoma


Indications for an Ostomy
- Congenital anomalies
- Bladder, colon, or rectal cancer
- Inflammatory diseases (Crohn’s, ulcerative colitis)
- Inherited disorders
- Ureter obstruction
- Stabs/gunshot wounds to the abdomen


Colostomy: colon (permanent)
Ileostomy: ileum (temporary or permanent)
Urostomy: urine (permanent)


Colostomy Types
- End colostomy: only one opening
- Loop colostomy: usually on transverse intestine; emergency procedure/intervention due
to obstruction
