Interdisciplinary Plan Proposal for Improving Handoff Communication
Objective and Predictions
This interprofessional communication and role operation change proposal is to
establish a uniform handoff communication process within all departments in the
Metropolitan Medical Center, with emphasis put on the handoff of information between the
ICU and other departments. From the interview conducted with Sarah Johnson, an ICU nurse,
there is no standard system of handoff practices that can be sampled, implying that more time
is wasted on communication gaps and even possible gaps in patient care. The goal is to use a
specific tool of handoffs like I-PASS (Illness severity, Patient summary, Action list, Situation
awareness and contingency plans, Synthesis by receiver), which is expected to enhance the
transfer of essential information about the patients between the shifts and departments
(Franco Vega et al., 2023). This implementation will likely decrease the incidence of
medication errors, increase the cohesiveness of care administered, and improve the patient's
safety in less than six months. It has quantifiable short-term goals; for instance, the
percentage of adverse events that are linked to communication errors ought to be reduced by
50%, and staffers' satisfaction rating of handoff practices should improve by 25% (Blazin et
al., 2020).
Change Theory and Leadership Strategy
As for the steering of this interdisciplinary undertaking, Kotter's 8-Step Change
Model will be adopted as the paradigm for change in the organization. Such a change model
is most effective when applied to big-scale changes that may affect several departments
simultaneously, for example, the implementation of handoff protocol templates (Miles et al.,
2023). The latter eight steps give understanding and structure for the actual process of putting
