How long is an independent contractor registration valid for? - ANSWERS-one
year, and becomes dormant on the last day of the month one year from the
date of issuance or renewal.
Application requirements for independent contractor registration - ANSWERS-a
completed application, proof of being 18 years of age, proof of holding a
current valid practitioner certificate, a current copy of the assumed business
